The medical your governing body asks for
Many sports ask for a doctor's signature before you train or compete. Divers who answer yes to certain questions on the diver medical questionnaire need a doctor's evaluation before they start a course. Motorsport UK asks some licence applicants for a doctor's medical, an ECG or a vision test, depending on the licence and their age. Cycling, event and expedition organisers set their own forms.

Sport medical questions
Recreational diving medicals, on the diver medical forms used by PADI, BSAC and the other training agencies. Commercial and professional diving medicals must be carried out by an HSE Approved Medical Examiner of Divers, and we will point you to the HSE list.
